Who is the Consumer Choice Association (CCA)?
The Consumer Choice Association (CCA) provides goods, services and information to consumers who may not have access to these programs through an employer. Consumers can join the association by paying the required membership fees

Is the CCA available in every state?
The CCA is available nationwide, except in Oregon and Florida. We hope to expand into these states soon.

How much is membership in the CCA?
Membership is just $18.00 annually.

How do I pay for the membership?
CCA requires payment annually. You can pay for your membership by check, credit card or bank draft (ECHECK).

Will my membership be renewed automatically every year?
No. You'll be notified 30 days prior to your anniversary date to re-enroll in the association and continue participating in the programs.

What information will I receive as part of my membership?
You'll receive a confirmation e-mail. Then, you'll receive a Program Brochure in the mail including a summary of the services available and an ID card(s) showing your actual effect date.

You can expect a Program Brochure within 10 business days from the date you submit your application with payment. If you do not receive a brochure, call Careington at 1-800-785-0699.

When and how do I use my discounts?
You may use your benefit as soon as you receive mailed confirmation that your program is effective. Refer to your ID card for your effective date.

Do I need an ID card?
Yes. CCA requires ID cards to access services and discount programs. You’ll need to provide your member ID number to participating providers when receiving services.

How do I re-order a replacement ID card?
Careington, our plan administrator, distributes member ID cards. Contact Careington at 1-800-785-0699 for more information.

Why did I receive a newsletter from the association?
The association is required to provide their members with a newsletter once a year. The newsletter introduces new program information, summarizes current programs available and notifies you of the annual meeting of members.

Where is VSP's Signature Choice Plan for Individuals available?
VSP's plan is available nationwide, except for the states of Oregon and Florida. We hope to expand into those states soon.

Do I have to join CCA to enroll in VSP's vision plan?
Yes. In order to enroll in VSP's Signature Choice Plan for Individuals, you must first join CCA for just $18.00 annually. Membership in CCA automatically provides you discounts on health, travel and business-related services-saving you hundreds of dollars every year.

How do I pay for the VSP plan?
You pay for the VSP premium annually, either by check, credit card or bank draft (ECHECK).

Can I purchase one plan for myself and a separate plan for my dependent(s)?
Yes. However, you need to complete two applications-one for each plan-and the "member only" rates apply to each plan.

How long does it take before I am enrolled in the program?
Your benefit is typically effective on the first of the month following receipt of your completed application and payment of premium. Please refer to your ID card for your actual effective date.

How long am I covered for with VSP?
Since VSP's Signature Choice Plan for Individuals requires annual payment, you're covered for one full benefit year.

How do I use the vision benefit?
Using your benefit is easy.

  1. Locate a VSP doctor near you at "Find a Provider" on this Web site or call CCA at 1-800-785-0699.
  2. Make an appointment with the VSP doctor of your choice and tell them you're a VSP member.
  3. At your appointment, identify yourself as a VSP member and present your ID card. Your doctor and VSP will handle the rest.

Can I see any VSP doctor?
As a VSP member, you can see any one of our 24,000 network doctors nationwide. VSP doctors are located right where you need them - close to work, home and shopping centers. And, most doctors offer evening and weekend hours and accept walk-ins.

Who is Careington International Corporation?
Careington International Corporation (CI) is the administrator for the Consumer Choice Association, as well as a third party administrator for VSP. CI is an expert in delivering individual medical, dental and health programs.

Can I sign up for the Careington Dental Plus program?
Yes. You can purchase Careington Dental Plus program as a standalone or combined with VSP and/or CCA for an additional fee. You'll save even more on programs and services-saving you hundreds of dollars every year.

Is this an insurance plan?
NO. Through your Careington membership, you receive a discount for the health services you require and pay as you go. There are no administrative forms involved.

How does the plan work?
Members who join may visit an "In-Network" provider and receive a discount off normal fees. The member pays out-of-pocket at the reduced rate.

What doctors can I go to?
You can review the list of participating providers in your area by using the provider search on this web site or by calling us toll-free at 1-800-785-0699. Please call to verify participation of doctors before you schedule an appointment.

Can I go to a provider that is not in the network?
No. You will only receive a discount by going to a provider in the network.

There is no provider close to my home. What do I do?
While some zip code areas may have a limited number of providers, there are usually providers within a 50-mile radius of your zip code.

If my dentist refers me to a specialist who is not in the network, do I still get a discount?
No. There are no out-of-network discounts.

How do I sign up?
It's easy. Join online or by calling us toll-free at 1-800-785-0699.

How long will it take me to get into the system after I sign up?
Within 48-72 hours after you join, your information will be downloaded into our system.

How do I change my address if I move?
Call our Member Services Department with your updated information toll-free at 1-800-785-0699, press option 1.

How do I file a claim for Careington's Dental Plus programs?
There are NO administrative forms to file. This is NOT insurance. You receive discounts from the health services you require. We are NOT a traditional indemnity insurance company.

Can I use this plan even if I already have insurance?
Yes, as long as your plan isn't a PPO, HMO, or another discount plan. Just go to a provider on the plan, show your card, pay the discounted fees, and ask for a copy of your bill. Then, file the claim with your insurance company to be reimbursed by them. This lessens your out-of-pocket expenses and stretches your dollar so you reach your maximums more slowly.

Can I include family members in my plan?
Yes. Select the Family Plan, and everyone currently living in your household will be eligible for discounts with the plan.

What do I do if I feel the provider overcharged me?
Write us right away! Send a copy of your bill and a written letter to:

What do I do if I have a complaint about a provider?
Write us right away! Submit all issues in writing including a copy of your itemized bill. The dental office refers to this as a 'superbill'. You may fax these to 1-972-335-3968, or you may mail them to Careington, Attention: Member Services, 7400 Gaylord Pkwy, Frisco, Texas 75034. Call Member Services at 1-800-785-0699 for help with any concerns you may have.
